- Should have registered with the Tamil Nadu Nurses and Midwives Council, Chennai.

- HSC / Pre-degree / AISSCE / ISCE / CBSE / Intermediate mark statement(s)
- Diploma Certificate.
- Diploma mark statements.
- Migration Certificate for the students who have passed Diploma course from states other than Tamil Nadu.
- Council Registration Certificates obtained from the Tamil Nadu Nurses and Midwives Council, Chennai
- Eligibility Certificate obtained from The Tamil Nadu Dr. M.G.R Medical University, Chennai
- Community Certificate (except OC candidates)
